TRANSLATION (in part):

In reference to your letter dated May 6th that you have lost your documents showing that you had returned to Italy only to visit temporarily. If you wants a new valid passport, we will need a certificate of birth, certificate from the police, two photographs, and a doctor’s certificate attesting that your illness suddenly appeared and that you lost your passport while in Italy.
